You will be required to submit information as part of the referral.

We recommend you gather the information in advance, or have access to staff who can provide it.

To support you in completing the referral, we have published an overview of the information required.

Children and young people with Education, Health and Care Plans (EHCPs)

Referrals can be made for children and young people with EHCPs. The referral must be made to the local authority responsible for maintaining the plan and its provision.

If Birmingham local authority have responsibility for the child or young person's EHCP, you may use this form to make a referral - even if the child or young person does not ordinarily and mainly reside in Birmingham.

If another local authority has responsibility for the child or young person's EHCP, you must contact them. They have responsibility for the Section 19 provision.

Consent

You must inform parents / carers that a referral is going to be made.

They must understand the details will be shared with other services - including education, health and care - as part of the process to determine any necessary Section 19 package of support.

Funding

The school may be required to transfer the AWPU and per pupil funding to the local authority for each week that the pupil is receiving tutoring or Alternative Provision funded by the local authority under their Section 19 Education Act duty.

Legal attendance route

If you believe that a legal attendance route may be needed, you must contact the council's Attendance Legal Intervention team before making a referral. Email attendance@birmingham.gov.uk.

Use of data

The information that you provide to us will be used to consider your referral and for subsequent administration.

It may also be used for management information and statistics to assess the service and measure future provisions.
