New applicants
We are not currently accepting new applications for drivers unless applicants have already completed a medical and obtained a DBS certificate through our occupational health service.

Driver renewals
You can complete the driver renewal application form, via the link below, and submit your application with the relevant attachments (listed on the form) to licensing@Birmingham.gov.uk. You must include in the subject line of your email what the application is for e.g. PHD renewal and if you have one, the date of your appointment.
Existing drivers
If you have an expired medical certificate or DBS, complete and submit the driver renewal application below as this now requires you to make a declaration of fitness/no new convictions.
New operator applications
You can complete the Private Hire Operators Licence Application form, via the link below, and submit your application with the relevant attachments (listed on the form) to licensing@birmingham.gov.uk. You must include in the subject line of your email what the application is for e.g. PHO application.
Operator renewals

Vehicle renewals
You can complete the vehicle renewal application form, via the link below, and submit your application with the relevant attachments (listed on the form) to licensing@Birmingham.gov.uk. You must include in the subject line of your email what the application is for e.g. PHV renewal.
New vehicle applications/transfer of vehicle licence
You can complete the vehicle application form, via the link below, and submit your application with the relevant attachments (listed on the form) to licensing@Birmingham.gov.uk. You must include in the subject line of your email what the application is for e.g. new PHV application.
You will also need to ask your insurer to email us direct with a copy of your insurance certificate.
If your vehicle is a new hackney carriage vehicle not manufactured by LTI or LEVC then you will be required a certificate of conformity. This can be a scan if you have the certificate or an email copy from the manufacturer of the vehicle.
New private hire vehicles must be younger than eight years (from date of first registration) when the application is submitted for licensing. A vehicle older than eight years will not be licensed. The age of imported vehicles will be assessed from date of first registration in the country of origin if that is earlier than the date of first registration in the UK.

Notify a change of address (driver/vehicle licences)
Complete the notify change of address form, via the link below, and submit your completed form to licensing@birmingham.gov.uk. You must include in the subject line of your email what the form is for e.g. change of address PH Driver

Report a lost/stolen licence (badge/plate)
Complete the report lost/stolen form, via the link below, and submit your completed form to licensing@birmingham.gov.uk. You must include in the subject line of your email what the form is for e.g. report stolen HC Plate
Applications will NOT be accepted unless all parts of the form are completed in full, and the relevant documentation provided. There will be a short waiting time in order to process your request.
