Three offences relating to conditions at Papa Johns, unit 2 Masshouse Plaza, Masshouse Lane, Birmingham, B5 5JE. There was mouse activity throughout the premises. The premises was not kept clean and maintained in good repair and condition in that floors, shelving, the toilet room wash hand basin, sink sealants and plastic containers holding sauce pots were dirty and at least one tap was in disrepair. Equipment which came into contact with food was not effectively cleaned in that there was a mouse dropping on a pizza dough tray.
